New Employee Forms

Welcome to Washington County

New Employees must complete the following forms and submit them to Human Resources on the first day of employment. Before your first day, complete the following items: 

OPTIONAL: PERA Life Enrollment Form - (Complete this form only if electing this additional coverage;  See PERA Life Brochure for additional information) 

On your first day, bring the following to Human Resources: 

  • The completed forms listed above AND
  • Your government issued identification to verify your eligibility for employment
